Description

AppleCare Medical Management Group was incorporated in 2003. Over the last 17 years, the multispecialty medical group of independent practices has expanded into new communities, contracted with more hospitals and health plans, and added over 50 new doctors - but the mission has remained the same. Today, AppleCare Medical Management is comprised of hundreds of board-certified private-practice, independent physicians. Collectively, our physicians have been serving the Southeast area of Los Angeles and Northern Orange County for decades, providing their medical expertise and passion to improving the health of our communities.

 

The Primary Care Physician (PCP) is responsible for the day to day clinical care of patients. The PCP manages the health of patients by providing preventive evaluations and wellness education, and treatment of acute and chronic illnesses/conditions. The PCP is responsible for the coordination of care with specialists and appropriate ancillary services.

 

Primary Responsibilities:

  • Observe patients (e.g., body language; interactions with others; behaviors) in order to evaluate their complaints
  • Listen critically to and ask clarifying questions of patients regarding their complaints
  • Perform appropriate physical exams and assessment of vitals in order to gather objective information about patient findings
  • Synthesize patient medical history and physical examination findings, and evaluate psychosocial needs, in order to formulate potential/differential diagnoses
  • Order/interpret appropriate tests in order to formulate/confirm diagnoses
  • Create/communicate/negotiate treatment plans based on the working diagnoses
  • Implement treatment plan (e.g., prescribe medication; order physical therapy)
  • Ensure appropriate monitoring, follow-up, and ongoing reevaluation of treatment plan
  • Apply evidence-based medical guidelines to provide effective clinical care (e.g., conduct appropriate testing for preventative care; follow disease specific guidelines)
  • Perform indicated health care assessments (e.g., Annual Wellness Exam or other group or plan related assessments) to identify all clinical conditions and treat accordingly
  • Refer appropriately to specialists as required to provide consultation and/or treatment for conditions that require additional technology or specialized experience
  • Provide timely, appropriate, and clear documentation to support care coordination and communication using relevant tools/systems (e.g., EMR)
  • Facilitate serious illness communication and advanced care planning discussions
  • Participate in other clinic based programs as needed
  • Communicate medical information in clear terms patients and internal and/or external care team members can understand
  • Seek acceptance from patients on diagnosis and treatment plan by listening to their concerns and explaining diagnosis, benefits of treatment plan, and next steps
  • Lead and/or participate in team-based collaborative delivery of patient care (e.g., work with health-care colleagues to facilitate delivery of patient care)
  • Educate/update internal team members/colleagues to facilitate implementation of new policies and procedures
  • Advocate to relevant stakeholders (e.g., senior leadership; internal business partners) regarding internal issues, challenges, barriers, and needed changes
  • Respond to requests/communications (e.g., pages; emails) from patients/caregivers and care team members in a timely manner to facilitate safe and effective patient care
  • Display appropriate levels of sensitivity, care, and empathy when listening to and discussing patients' concerns
  • Apply understanding of patient care quality metrics (e.g., HEDIS; 5 STAR; disease-based metrics) in order to drive/achieve quality outcomes
  • Identify and/or share best practices to drive improvements to medical/clinical performance
  • Participate in clinical programs, workgroups, and committees to drive clinical quality and business outcomes (e.g., physician retention; quality review; best practices; site based programs)
  • Apply understanding of appropriate resource measures (e.g., appropriate location for care; generic medication; access) to drive effective business results
  • Apply accurate diagnostic codes in order to maximize risk adjustment
  • Maintain awareness of financial performance drivers for relevant business unit and strive to support financial success
  • Monitor clinical performance metrics (e.g., admission/readmission rates; length of stay; CHF; COPD) in order to identify opportunities to reduce health-care costs
  • Comply with medical ethics guidelines applicable to one's area of specialty
  • Maintain appropriate certifications/licensure applicable to one's area of specialty
  • Maintain appropriate hospital/institutional/insurance credentials
  • Demonstrate and maintain technical skills/knowledge consistent with community standards within one's area of specialty
  • Participate in ongoing continuing medical education
  • Supervise clinical staff (e.g., mid-levels; staff nurses) and comply with state regulatory and professional standards in performance of duties
  • Maintain awareness of and comply with relevant organizational policies, procedures, and regulatory requirements (e.g., HIPAA;Medicare Fraud Waste & Abuse)
  • Participate in office/clinic training and oversight
  • Complete all medical records timely
  • Report any identified issues/situations to the appropriate authority/government agency, i.e. Adult Protection Services (APS), Children’s Protective Services (CPS), etc.  Act as the patient’s advocate in coordinating the use of the entire health care system to best serve the patient.
  • Follow OSHA Standards and Universal Precautions
  • Maintain quality of care standards as defined by ACHG or other accepted boards/organizations
