Keystone Center is seeking a Case Manger. Keystone Center is located in Delaware County, PA.  Keystone Center offers substance use disorder and dual diagnosis treatment for adults.  We are located five miles south of the Philadelphia Airport, making our facility an easy location to travel to and from.

The Case Manager is responsible for monitoring and delivering high quality service to dually diagnosed/chemically dependent patients.  Primary responsibilities include: Coordination of the  full continuum of care; Assessment and documentation of clinical needs; Implementing and delivering individual, group and educational services; preparation of PCPC reports.  The provision of a safe and therapeutic milieu; Crisis intervention; and full participation with the multidisciplinary team toward meeting a patients' treatment plan objectives.  Discharge planning and referrals to community resources required.

Job Specifications:

To perform this job successfully, an individual must be able to demonstrate competency in the criteria listed in this job description. The specifications listed below are representative of the knowledge, skill, and ability to meet the minimum requirements for this position. Staff must demonstrate ability to provide clinically appropriate care to varied populations, including chemically dependent and dually diagnosed adult patients. Facilitate all clinical interventions and document fully.  Work with internal resources and departments to facilitate treatment.

 

Education and Experience:               

A Masters level mental health professional (with a degree in a related mental health discipline from an accredited college) with one year full-time experience in a chemical dependency treatment setting

             

 

Knowledge:

Must possess knowledge of dual diagnosis / chemical dependency treatment approaches, 12 Step Program recovery philosophies, basic counseling group processes, therapeutic relationships, age-specific growth and developmental conditions impacting treatment, limit setting and behavior management, crisis prevention and intervention techniques, understanding of funding sources, and community resources.

Environmental Conditions:

Professional office environment. Exposure to dually diagnosed patients who potentially may exhibit violent or aggressive behavior. Potential for exposure to communicable diseases, blood/bodily fluids.

 

Physical Requirements:         

Ability to communicate verbally and in writing. Physical agility to manage patients. Push, pull and lift up to 40 pounds. Intermittent walking, bending and stooping. Drug and alcohol free as evidenced by negative results of post-offer, pre-employment urine drug screen.
