Valley Hospital is seeking a PRN (as needed) Treatment Plan Coordinator.

The Treatment Plan Coordinators develop and updates the patients Plan of Care. With input from patients, families, and the treatment team, the Treatment Plan Coordinator creates outcome goals and an overall plan of care. The Treatment Plan Coordinators works with all staff members to ensure the plan is implemented correctly. Develops and Implements the patient’s plan of care with specific and measureable goals, objectives and interventions defining actions unique to each patient’s need. Develops interdisciplinary Plan of Care by participating in treatment team meetings; coordinating information and care requirements with other care providers. Prioritizes and formulates a plan of care based on a patient’s assessment

Valley Hospital is centrally located with easy access to many areas of greater Phoenix. We are between North 35th and 36th Streets with East Earl to our north and East Thomas to our south.

Please note this is a non-benefit eligible position.

Job Requirements

Education:  Masters degree in Social Work or Counseling, or related field required.

Experience: Experience in patient care diagnosis, treatment plans, case management and discharge planning preferred. Experience in the mental health field preferred. A thorough knowledge of TJC, Medicare, OSHA regulations and all other state and federal behavioral health regulatory standards.
