Foundations for Living is situated on 14 rolling, wooded acres in Mansfield, Ohio, Foundations For Living is a secure residential treatment center for youth 11 to 18 years of age. Our privately funded, 84-bed center (14 males, 70 females) is accredited by CARF and licensed by the Ohio Department of Mental Health & Addiction Services (ODMHAS). We offer proven, pro-social programs that capitalize on the strengths of adolescents with emotional or behavioral problems.


We are currently recruiting for a Qualified Mental Health Specialist. This position is scheduled to work Sunday thru Thursday from 8:30AM-5PM. Primary responsibilities may include: Completes a life skills assessment upon client admission and prior to discharge. Develops life skills goals for residents in conjunction with the treatment team. Initiates contact with caseworker/legal guardian upon admission. Meets with client for a minimum of 30 minutes every other week to address life skills goals, phone calls and basic needs needed, to review progress on unit, identify barriers to QMHS treatment goals. Completes progress notes related to QMHS/case management services. Meets with treatment team weekly. Coordinates family visitation for in state and out of state clients. Coordinates family education as needed. Conducts educational groups with residents. Maintains weekly contact, more as needed, with caseworker/legal guardian. Assesses, manages and links with resources to basic needs including food, clothing, education, housing and vocation. Accesses and assists in the utilization of community resources for clients. Represents and coordinates residents in any legal issues. Attends court appointments and works court appointments around his/her schedule.

Job Requirements

Minimum of a Bachelor’s Degree in Social Work or related field.

Must be at least 21 (twenty one) years of age.
