The Staff Nurse (R.N.) is a registered professional nurse who prescribes, coordinates, and evaluates patient care through collaborative efforts with health team members in accordance with the nursing process and the standards of care and practices.

Job Requirements

Education: Graduate from an accredited program of professional nursing required; Bachelor's Degree preferred.

Experience: A minimum of three (3) years experience in a psychiatric healthcare facility preferred.

Licensure: Currently licensed to practice by the Texas State Board of Nursing.

Additional Requirements: Must pass a medication administration exam before administering medications. CPR certification and successful completion of Handle with Care and Verbal De-escalation including restraint and seclusion policies, within 30 days of employment and prior to assisting in restraining procedures.

STANDARDS OF PERFORMANCE

THE ESSENTIAL JOB FUNCTIONS ARE CATEGORIZED UNDER THE JOINT COMMISSION  STANDARDS OF PERFORMANCE.

 

  1. Assessment:
  2. Completes a comprehensive nursing assessment and patient interview according to hospital policy.

  3. Documents patient information and nursing care in the provision of nursing services.

  4. Identifies patient problems based upon data collected through nursing assessment as well as integrating information obtained through family, significant other, and/ or primary caregiver.

  5. Establishes priorities based on nursing assessment, problems identified, and estimated length of stay while developing short and long term goals and desired outcomes on an individual basis.

  6. Utilizes accurate and thorough documentation using appropriate terminology and approved abbreviations.

  7. Documents and reassess any significant changes in the patient’s condition in compliance with hospital policy.

  8. Assesses vital signs and reports abnormalities to the physician.

  9. Reports any signs and symptoms of infections to the Infection Control Nurse.

  10. Verifies the accuracy of patient records.

  11. Makes assignments based on patient’s needs.

     

  1. Intervention:
  2. Employ principles of communication, interviewing techniques, problem solving and crisis intervention when performing psychotherapeutic interventions.

  3. Intervention is based on knowledge of psychiatric and medical nursing procedures, as well as the Master Treatment Plan (MTP).

  4. Provides appropriate intervention related to patient’s concerns.

  5. Accurately administer medication to patients.

  6. Reports any medication error and accurately completes occurrence report form.

  7. Demonstrates safe storage and control of medications while accurately maintaining the Medication Administration Record (MAR).

  8. Verifies informed consents have been obtained prior to administration of medications.

  9. Evaluates and documents the patient response to medication.

  10. Transcribe and carry out physician’s orders accurately.

  11. Identifies patients at risk and implements procedures to monitor and reduce at risk factors.

  12. Utilize appropriate interventions in psychiatric and medical emergencies.

  13. Accurately and thoroughly documents nursing interventions daily using goals developed in the MTP.

  14. Safely uses and maintains biomedical equipment (blood pressure machine, crash cart)

  15. Maintains a therapeutic relationship and observes appropriate staff/ patient boundaries.

     

  1. Leadership:
  2. Ensure that all staff members deliver care in accordance with the Standards of Care and Practice.

  3. Makes assignments that reflect the competence of staff and patient needs. Ensures that an assignment sheet is filled out every shift.

  4. Implement hospital policies and procedures and act as a resource to the staff.

  5. Responsibilities for compliance with the Joint Commission, CMS, state and regulatory standards and patient rights standards.

  6. Ensure Infection Control, Safety and Risk Management policies and procedures are consistently implemented.

  7. Coordinate, maintain and evaluate the effectiveness of treatment planning offered.

  8. Complete occurrence reports when indicated.

  9. Participates in peer review committee and performance improvement activities.

  10. Monitor medical records for appropriate documentation of care and effectiveness.

  11. Ensure that patients receive timely quality care.

  12. Recognizes own limitations, seeks information and advice when unable to solve conflicts/ problems on an individual basis.

  13. Assists to orient new nurses and MHTs to the unit and job responsibilities.

     

  1. Treatment Planning
  2. Prioritize and formulate a plan of care based on patient assessment and according to policy.

  3. Participate in the development of the MTP with specific and measurable goals, objectives and interventions defining actions unique to each patient’s needs.

  4. Update and revise plans as goals/ objectives are met or when the patient’s condition changes.

  5. Document both the nursing interventions and patient’s response to treatment.

    Milieu

  6. Assure patients are adequately oriented to the program.

  7. Monitor program activity and encourage patient participation.

  8. Prescribe and coordinate nursing care to patients, during episode of care.

  9. Provide structure and maintain a therapeutic environment in collaboration with the patient and other health care providers.

  10. Consistently utilize universal precautions and environmental safety guidelines.

  11. Conduct structured didactic groups with patients and families.

  12. Utilize self as a role model to promote health communication.

    Teaching

  13. Provide education to patients and families based on their identified needs and limitations, such as, communication, interpersonal and social skills as well as basic principles of physical health.

  14. Provides and documents discharge instructions including food and drug interactions, diets and activities as appropriate.

  15. Evaluate and document the effectiveness of all patient teaching.

  16. Provide opportunities for patients and families to question, discuss and explore their feelings about prescribed therapies/interventions/diagnosis.

    Performance

  17. Participate in quality assurance programs.

  18. Collaborate with other team members.

  19. Act as the patient's advocate and assure that patient rights are upheld.

  20. Comply with established measures for infection control, OSHA regulations and risk management.

  21. Utilize time constructively and in an organized manner to accomplish assigned responsibilities.

  22. Demonstrate guest relations in communication, cooperation and in giving assistance to patients, physicians/licensed practitioners, co-workers and visitors in a prompt and courteous manner.

  23. Follow facility, departmental and personnel policies and procedures

  25. Works overtime in a case of emergency or as deemed necessary by supervisor.

  26. Attends mandatory educational programs and annual in-services.
