Job Description

Reporting to the Director of Critical Care Services this position assumes the accountability of all aspects of adult and geriatric patient care utilizing the Nursing Process and within the guidelines of the State of California Nurse Practice Act. Accountable for maintaining standards of nursing practice and optimal patient outcomes in the ICU. Responsible for the supervision of all categories of nursing personnel.


 Essential Job Duties:

Assessment Completes a thorough history/assessment on all assigned patients taking into consideration the physiological and anatomical changes that occur with aging. Identifies those that are at risk for falls, decrease in skin integrity, etc.. Assesses patient changes in care, acuity, needs, and reports changes to charge nurse.

Planning Observes, identifies, and collects data to identify nursing care needs of the patient. Geriatric patients with their individual needs an aging process are reflected in the plan. Psychosocial needs of the patient/family and significant others are part of the plan. Involves all disciplines as needed.

Initiates comprehensive care plan following full assessment.

Implementation Provides quality nursing care based on patients' needs and plan of care. Supervises health care given by other nursing personnel.

Performs procedures safely to the geriatric patient taking into consideration their limitations due to advanced age.

Patient Education Plans and documents patient and/or significant others' educational needs and documents intervention and patients'/significant others' understanding in nursing notes and/or education flow sheet.

Utilizes individualized teaching techniques when necessary in working with patients experiencing sensory or learning deficits.

Discharge planning includes appropriate referrals and assistance in coordination with the case manager.

Safely administers medications to the geriatric patient taking into consideration their decrease in absorption, swallowing ability, etc.

Discharge assessment and instructions are completed and signed by the patient. Any unusual occurrence is documented on the incident trending report.

Upholds the standards of patient care in a safe, efficient, and cost effective manner.

Responsibilities to the Medical Staff Communicates significant changes in patient conditions and abnormal diagnostic data requiring medical attention.

Responsibilities to the Unit/Other Departments: Demonstrates a basic theoretical and practical knowledge of the management of the critically ill patient. Exhibits knowledge of skills required to implement safe nursing care and health promotion of the hospitalized patient demonstrated through practice.

Evaluation and Documentation reflects the plan of care and the activities of the patient care per physician orders. Care plan is updated and resolutions are documented.

Medication Administration Demonstrates safe and accurate administration and documentation of medication including administration of blood products.


Job Requirements


Job Requirements:


  • One year ICU/StepDown/PCU experience preferred.


  • Current California RN license required.

  • Basic Arrhythmia Certificate or 12-Lead EKG Certificate preferred.

  • ACLS required within 90 days of start date.

  • Current BCLS (CPR) required. CCRN preferred.
