Job Description

Texas NeuroRehab Center (TNC) provides complex medical and brain injury rehab, chemical dependency treatment for adults and neuropsychiatric treatment for youth. The capabilities of the facility as a whole allow for integrated and specialized care for children, adolescents and adults facing difficult challenges.

Texas NeuroRehab Center is currently accepting applications for a Registered Nurse (RN). RNs provide direct patient care and medical management and assessment of the patients. A Registered Nurse ensures environmental safety and crisis management for the patient population, and directs the implementation of behavior management program to ensure consistent carry-through.

Duties and Responsibilities
Completes nursing admission assessment of the patient within 8º.
Pain, nutrition, and safety issues are assessed during admission & if identified as problems are included as nursing diagnoses in the nursing care plan.
Identifies nursing diagnoses (to include knowledge deficit, as well as pain & potential for injury if identified), writes the Nursing Care Plan, and obtains the physician’s signature within 24º.
Ensures that the patient orientation to the unit is completed within 8º, to include:
Inventory of possessions, explanation of rights, pain management, and safety.
Assesses and documents findings when patient condition changes.
Communicates changes in patient status to physician and treatment team.
Documents patient progress clearly, accurately, legibly, and concisely.
Ensures pain is addressed each shift, if indicated.
Ensures unusual events are thoroughly documented. Initiates additional reports as needed (Staff injury, patient injury, etc.). Records behavioral data in prescribed format and includes this data in narrative. Educates staff on documentation-related issues.
Ongoing patient assessments reflect the full range of needs, including: cultural, spiritual, growth and development, initial status, changes, and progress in all areas, including pain, potential for injury, and knowledge deficit.
Documentation in the medical record is to be timely, accurate, and reflective of facility policy.
Nursing Care Plan Updates are completed accurately, based on a review of the patient’s progress and needs, and on time.

Job Requirements

Knowledge of appropriate nursing assessment, diagnoses, and interventions with patients
Knowledge of milieu management
Ability to direct subordinates in the performance of their duties
Ability to accurately administer medication and monitor side-effect
Ability to safely implement Special Treatment Procedures
Ability to develop therapeutic relationships with patients and families
Skill in preparing and maintaining appropriate medical record documentation

Education and Training
Graduate of an accredited school of nursing
BSN preferred

Licensure and/or Certification
Current State of Texas Registered Nurse License
Current CPR certification
RN,C; CRRN preferred

One year of inpatient medical surgery, rehabilitation, or psychiatric experience preferred

Able to sustain physical activity throughout scheduled shift
Able to remain on feet throughout scheduled shift
Able to physically restrain patients by individual or team restraint per policy
Able to lift, carry, and lower 50 pounds without assistance
