Job Description

Assessment: Performs Patient Assessment According to Hospital/Unit Policy

  • Performs initial assessment appropriate to the patient’s clinical presentation/diagnosis, care setting, age, development level, and cultural and religious orientation in a timely manner as it pertains to diagnostic testing/scheduled procedure.
  • Performs completely and accurately including, but not limited to, physical (i.e. assessment for pain, skin breakdown, risk for falls), psychological (i.e. cognitive and communicative skills or development), social status and other patient needs prior, during and post invasive diagnostic testing/ procedure.
  • Identifies and prioritizes the need for further assessment based on the patient’s clinical presentation/diagnosis and tolerance of diagnostic testing/scheduled procedure.
  • Reports abnormal findings or values to the physician(s)
  • Collaborates with the anesthesiologist and specialist



Planning: Develops a Patient Plan of Care

  • Is individualized for each patient based on assessment of needs that include input from the patient, family and other multi-disciplinary health care team members.
  • Pre-screens patient lab work, mobility, allergies, and medication history prior to invasive diagnostic testing/ procedure.
  • Identifies specific nursing diagnosis is relevant to assessment findings. Appropriately notifies physician if scheduled invasive diagnostic test or procedure is impacted by assessment.
  • Identifies interventions specific to identified patient needs during invasive diagnostic testing/ procedure.
  • Includes plan for discharge/transfer

4.  Implementation

  • Performs interventions identified specific to the patient’s individual needs to ensure safe, successful invasive diagnostic testing/ procedure.
  • Demonstrates the ability to effectively communicate patient care needs to all members of the multidisciplinary health care team.
  • Conducts patient/family education that:
    1. Is appropriate to the patient’s age, learning needs, level of understanding and readiness to learn.
    2. Includes the family/significant others when appropriate.
  • Documents nursing intervention and patient teaching.

5.  Evaluation 

  • Evaluates and documents the patient’s response to the invasive diagnostic testing/ procedure.
  • Revises and updates the Plan of Care as evidenced by documentation.
  • Documents patient’s response to teaching.

6.  Critical Thinking Skills

  • Demonstrates sound judgment and effective decision-making in following the nursing process.
  • Triages/prioritizes multiple patient interventions.
  • Changes priorities based on new assessment data.
  • Identifies supplies, equipment, and additional personnel necessary to complete a given assignment.
  • Analyzes data collected, weighs alternatives, and acts appropriately.
  • Facilitates systems/processes in response to situations that have the potential to negatively impact patient/family outcomes.

10.  Unit Specific Competencies

  • Demonstrates the ability to utilize monitoring system, assess findings, intervene as indicated and evaluate.
  • Demonstrates knowledge of Moderate Sedation policy and demonstrates expertise in providing as   ordered.
  • Able to titrate drugs according to unit policy in response to changing patient condition.
  • Ability to assist with any related processing of documentation within the department.
  • Provide on-call coverage as needed by the department.
  • Demonstrates knowledge of the step-by-step progress of specific sterile procedures for all  special procedure examinations.
    1. Anticipates Radiologist's/ Cardiologist's needs in regards to instrumentation, sutures, sponges, supplies, etc.
    2. Displays dexterity in the use of the required instrumentation according to technique, procedure and special requirements.
    3. Demonstrates ability to function in Special Procedures / Cath Lab situations requiring in-depth technical expertise and knowledge of anatomy and physiology.
  • Anticipates instrumentation/equipment for a wide range of simple and complex procedures.
    1. Selects appropriate instrumentation, catheters, and supplies consistent with Radiologist's /Cardiologist’s preference.
    2. Sterile environment is set up and organized prior to patient's arrival into room.
  •   Able to circulate and document procedure
    1. Assesses patient prior to procedure reporting to physician all pertinent information.
    2. Assess patient’s pain, medicates as ordered and documents response.
  • Able to anticipate changes in patient and notify physician.
  • Demonstrates ability to conduct a time out at the beginning and end of the case utilizing the WHO universal protocol.
  • Ability to orient new staff to the department and act as a resource when necessary.
Job Requirements
  • Completion of an accredited Registered Nursing program.BSN preferred.

  • Five years critical care experience procedure experience preferred.

  • Current California RN license (maintain current)

  • BLS (maintain current AHA)

  • ACLS (maintain current AHA)

  • Use of current equipment.

  • Ability to communicate effectively and to foster teamwork among fellow staff members.

  • Ability to exhibit excellent written and verbal skills.

  • Attention to detail, high level of accuracy.

  • Ability to maintain confidentiality and function effectively under high volumes of patients.

  • Ability to exercise independent judgment on a regular basis.

  • Ability to define problems, collect data, establish facts and draw valid conclusions.

  • Knowledge of all interventional radiology procedures.
