**This position will work Friday-Sunday or Saturday-Monday.  Excellent compensation, benefits within 30 days, bonus potential, annual reviews.**
The RN Case Manager position is responsible for verification of appropriate incoming orders, completed orders and authorization of insurance benefits. Receive and respond to all incoming calls from referral sources or potential patients, exchange information to identify patients' needs, and determine the Company's ability to meet them. Enter referral information in system including clinical information and orders for services if available. Serves as the initial client contact to obtain patient demographics, type of referral, and type of insurance and insures the information is accurate. Responsible for staffing all new referrals, scheduling of patient visits daily per physician orders, review schedules often for any unassigned visits to be staffed and next day schedules. Will be responsible for weekend on-call for the home health and intake department.  
Primary Responsibilities:
  • Accepts and records initial client information to include type of referral, type of insurance, and patient demographics
  • Enter referral or order into EMR system both for home health
  • Verifies information with patient including demographics, insurance carrier, physician and pertinent information to help in the completion of the referral
  • Support team members of intake in reviewing orders received from physician office, hospital and case managers for accuracy and the supported documentation in order to serve the patient for home health
  • Follow up with referral source when questions arise in reference to an incomplete order or new referral
  • Develop and maintain a working knowledge of all services and resources provided by the Company and available within the community
  • Requests the referral source fax orders on all patients
  • Will receive report for weekend from intake and home health staff
  • Notifies pharmacy and home health when verification of new referral is complete
  • Ensures information inputted into the EMR system is accurate and complete to ensure timeliness of patient care, obtaining authorization for services requested   
  • Responsible for scheduling all physician orders, new referrals and schedules appropriate field staff member on a daily basis
  • Retrieves lab results from previous day
  • Responsible for scheduling per physician order and ensuring re-authorization department has updated order in order to obtain authorization from physician office
