The Ridge Behavioral Health System is seeking a dynamic and talented ECT Coordinator. Located in Lexington, KY, the heart of the Bluegrass, The Ridge Behavioral Health System offers a healing environment that is reassuring to a patients’ sense of security and confidentiality. Inpatient care is provided on a 24-hour basis, seven days a week. Outpatient and partial hospitalization services are provided on a six-day per week basis, days and evenings. Position Summary:

 

Provides structure and oversight to the day-to-day unit routines.  Engages the entire patient care team toward reaching common goals.  Leads the unit in providing professional nursing care to patients within treatment milieu utilizing Standards of Nursing Practice and Care.  Trains new nurses to unit and facility policies, procedures, practices and in developing unit specific training for nurses.  Implements performance Improvement as identified by nursing leadership.

 

Position Specific Competencies:

 

  • Accurately and timely completes the following duties: new patient admissions, assessment, orders, treatment plan, required testing, medication reconciliation, progress note, patient education, safe and efficient medication administration
  • Completes emergency transfer by performing verbal nurse to nurse report and transfer packet, insuring continuity of care and communication between levels of transition
  • Completes patient observations while ECT patient is in suite
  • Accurately and timely transcribes physician orders
  • Thoroughly completes patient Discharge Sheet
  • Demonstrates ability to administer medications according to hospital policy and procedures
  • Communicates changes in patient’s status (legal, psychiatric, physical) with physician and immediate supervisor and other ECT team members
  • Follows proper NCI de-escalation techniques when handling crisis situations.
  • Provides appropriate crisis intervention as necessary by anticipating and responding quickly to escalating behaviors, utilizing de-escalation techniques to diffuse the situation, correctly prioritizing safety concerns.
  • Uses appropriate counseling skills by de-escalating aggression, using active listening skills, using appropriate tone and volume of voice, providing a high frequency and variety of positive reinforcements, maintaining appropriate boundaries, and promoting a positive peer culture
  • Demonstrates proper procedures for therapeutic holds and seclusions
  • Contraband is properly removed and disposed of according to policy
  • Reviews documentation of nurses and other staff to insure depth of content, signature, date, time, credentials all complete and addresses identified issues
  • Assists in monitoring ECT unit staff mandatory training requirements are completed within time frames
  • Ensures completion & documentation of vital signs
  • Ensure therapeutic milieu is maintained
  • Participates and arrives prepared for all treatment team meetings
  • Ensures ECT staff completes duties as assigned
  • Synthesizes all aspects of patient care, ancillary services and how they interface with patient care, and identifies areas of needed improvement.  Contributes to clinical improvement process
  • Responds to all PI, QI, Infection Control and other indicators for the specific unit to improve processes for better outcomes
  • Completes Assignment Sheet:
  • Assignment of patient observations including delegation of Charge RN
  • Assignment of breaks and meal periods
  • Medication administration responsibilities
  • Group responsibilities
  • Assignment of code responsibility
  • Rounding with purpose, looking at all areas of the environment daily and correcting when necessary
  • Check if lab work has been ordered, samples collected, results have been received and placed in appropriate patients’ chart flagged for physician review
  • Ensures accuracy of ECT log daily
  • Ensures completion of Infection Control/Security checklist and IC processes are followed throughout shift by all staff
  • Ensures narcotic count is correct daily with staff signatures & emergency box is checked
  • Ensures MARS and all logs (i.e. Glucometer, Refrigerator and Unit Safety & Security logs) are reviewed for completion and accuracy during shift report
  • Notify Nurse Manager and/or Nursing Supervisor of any staffing needs, unusual events, risk indicators, & infection control issues.  Complete forms as required
  • Ensures chart audits are completed to comply with all pertinent regulations
  • Check Oxygen tanks’ gauge for O2 and ensure appropriate storage of tanks at all times
  • Notify parents/guardian of Seclusion/Restraints, patient injuries and unusual circumstances
  • Insures all Policies & Procedures related to ECT are followed by members of ECT team. (in all areas such as Infection Control, medication storage and handling, labeling of medications, wasting of medications, narcotics log, etc.)
  • Completes pre-certification, benefits verification, coordination of physicians and patients, and scheduling of ECT treatments
  • Provides or oversees pre ECT education and assessment is completed and documented for every patient (and involved family member) who is scheduled for ECT
  • Reviews pre-ECT physician orders to insure all diagnostic testing, assessments, and instructions have been followed or are on the chart prior to treatment
  • Insures appropriate Consent for Treatment is completed, patient has had opportunity to ask questions of physicians and ECT nurses prior to sedation, and signature is obtained prior to treatment
  • Maintains ECT equipment and supplies in good working order and at appropriate PAR levels, equipment checks are completed per policy. Ensures quality checks are completed as required
  • Assists anesthesia and psychiatrist during ECT treatment, monitors patient response and documents
  • Insures all aspects of care during the ECT procedure through discharge from ECT promote patient safety
  • Insures documentation of treatments for ECT patients is complete, signed off by appropriate disciplines
  • Accounts for narcotics and waste on narcotic sign-out sheet and/or other meds used for ECT procedures. Works with Pharmacist to maintain adequate supply of needed medications. Ensures expired medications are returned to Pharmacy
  • Interfaces with IP and OP departments/units regarding ECT patients’ status for pre- or post- care follow up, as needed
  • Insures all required Pt Healthcare Questionnaires, Assessments, and other benchmarking forms related to Patient Satisfaction are offered to every patient for completion
  • Oversees and assists in recovery of patients to insure safe, supportive care of patients through discharge. Assessment prior to discharge is documented thoroughly with discharge instructions provided to patient/family and documented
  • Documents nursing assessment response to treatment after procedure and when patients return for subsequent procedures, to include patient’s perception of care and effectiveness as well as nursing observations
  • Progress notes are current, complete, legible, and reflect accurate objective information and adhere to documentation protocols and policies.  Notes include physical, medical, and behavioral status in accordance with BIRP guidelines
  • Documents significant developments and/or changes in a patient’s behavior or condition promptly to unit staff and other team members as necessary
  • Documents intake/output assessments, if applicable
  • Documents & reports incidents immediately and according to policy
  • Documents Medication Education and patient perception of effectiveness & side effects 
  • Completes unit audits as assigned by the nursing leadership

Job Requirements

Job Qualifications:

 

Education

  • Graduate of an accredited school of nursing.
  • Bachelor’s degree preferred.

Experience

  • Two years of experience in psychiatric nursing preferred.
  • One year preferred experience in a Critical Care Unit, ER, or Recovery.  One year preferred experience as a Unit charge RN, Department Director, or Supervisor.

Certifications/Licensure

  • Current RN Licensure
  • NCI (nonviolent crisis intervention) required.  This training also offered upon hire.
  • CPR
  • ACLS current

Other Skills

  • Demonstrate accurate physical, mental, emotional and spiritual assessment skills.
  • Ability to clearly communicate findings both verbally and in writing. 
  • Demonstrate theoretical and practical knowledge of Standards of Nursing Practice and Care in all aspects of patient care provided. 
  • Demonstrate knowledge and support of the psychotherapeutic based treatment model and ability to utilize knowledge in patient care and therapeutic milieu.
