Position Description

Schedule: Monday - Friday, 8:00am - 5:00pm
 
Primary Responsibilities:
  • Manage the concurrent review of patients SNF Medical Record
  • Prepare transition of care plans for patients readiness for discharge
  • Attend SNF case management rounds
  • Meet face to face with patients and family
  • Manage the NOMNOC process with the SNF
  • Ability to travel to various assigned SNF facilities involved- 50% in Inland Empire area
Required Qualifications:
  • Current and unrestricted California RN or LVN
  • 3+ years of concurrent review in a managed care environment
  • Proficient in Microsoft Word / Excel
Preferred Qualifications:
  • Previous experience working in SNF
  • Strong clinical documentation skills
  • BSN
  • Bilingual
