Peachford Hospital is seeking a dynamic and talented Physician Practice Nurse.

Located in the beautiful Dunwoody neighborhood of Atlanta, Peachford features individual units for children, adolescents, adults and seniors, serving residents of Georgia and beyond. We offer inpatient acute care, as well as partial hospitalization and intensive outpatient programs. (www.peachford.com)

The Physician Practice Nurse will initially assess patients through a systematic process of interacting with patient, the patient’s significant others, and the appropriate health care providers.

Key Responsibilities include:

  • Collect and analyze data regarding the patient’s physical, psychosocial, environmental, self-care, educational, and anticipated discharge planning needs.

  • Demonstrate knowledge and ability to assess and provide appropriate interventions that are unique to the needs of the patient population (age and program specific) being served.

  • Coordinate treatment team meeting daily for assigned physician team.

  • Assist physician to coordinate patient plan of care, which includes rounding daily on patients, documenting findings and gathering pertinent clinical and UR information.

  • Communicate pertinent treatment and discharge plans to other team members.

  • Communicate changes in patient’s status (legal, psychiatric, physical) with attending physician.

  • Participate in facility QI process by auditing records/systems as indicated and communicating results and opportunities for improvement.

  • Be available by beeper or cell phone to assist in additional physician-related issues as assigned.

  • Coordinate with physician for assignment of patients to be seen.

  • Document progress notes and treatment plan.

  • Adhere to all safety policies and safe work practice.

  • Adhere to all hospital policies and procedures.

  • Perform other duties as assigned.

  • Support facility internal and external customer service standards. 

  • Follow Peachford Hospital Time and Attendance policy.

Job Requirements
