The Supervisor assists the Associate Director in developing and leading professional clinical and non - clinical staff in required care coordination activities which facilitate and support quality, cost efficient outcomes and minimizes fragmentation of healthcare delivery. The Supervisor has experience and expertise in the Case Management processes and coaches the staff on the RN - patient - physician collaborative process of assessment, planning, implementation, coordination, monitoring, and evaluation to address patients holistically. The Supervisor assists the Associate Director to support and coordinate the operations effectiveness across the continuum of telephonic case management. The Supervisor assists the Associate Director to help staff understand and demonstrate the ability to integrate the needs of members, the member and the members care team (community, providers, internal staff) regarding barriers to following the plan of care or other health information. This plan includes ongoing screening of the member for changes in health status and coaching for improved self-management.
Selected candidate must be willing and able to obtain and maintain multiple state licensure. (Application fees and filing costs paid for by UHG)   
*** This is a telecommute position in the state of Virginia and must be able to work the hours of 12:00pm-8:30pm EST Monday – Friday. ***
Primary Responsibilities:
  • Oversight of a team of clinical and non-clinical staff delivering services telephonically to individuals receiving LTSS services within the state of Virginia; Services include care coordination and planning, ongoing support and facilitation with the health care delivery team members
  • Work with the program Associate Director to identify and hire large volumes of new employees according to set timeframes per regional program roll-out
  • Actively participate in the implementation and planning prior to program roll-out to ensure staff readiness and flawless execution across each region
  • Reviews work performed by their assigned team, provides recommendations and coaching for improvement as needed and holds staff accountable to performance expectations
  • Motivates and inspires team members
  • Anticipates customer needs and proactively develops solutions to meet them
  • Serves as a key resource on complex and / or critical issues
  • Solves complex problems and develops innovative solutions
