Position Summary:

First Home Care is one of the largest Mental Health providers in the Washington DC Metro area. We provide services that enable youth, families and adults to receive the care they need in their homes, communities and schools, reducing the need for hospitalization and out-of-community placement. Our programs offer free-standing, independent levels of care. As such, they provide ancillary treatment to support programs of Social Service agencies, special education programs and office based therapies.

We are currently seeking an experienced School Based Community Support Worker for our DC location to work as a member of a unique mental health team providing services to Children and Families in communities, homes and schools.

Job Responsibilities:

• Assesses the mental health needs of clients and families as assigned.
• Provides or arrange for clinically appropriate prevention, early intervention, and treatment services, initiating outside service referrals as necessary.
• Completes intake protocol, diagnostic assessment designated pre and post treatment assessment on all clients on treatment caseload.
• Conducts individual, family, group counseling/therapy, and skill building, with clients and families.
• Provide home interventions/visits as at least once a month and more as clinically appropriate.
• Solicits parent/caregiver involvement in program planning and treatment services.
• Completes documentation as required by licensure and First Home Care in an accurate and timely manner.
• Contacts families within 48 hours of case assignment.
• Engages at least one time per month with all families, collaterals and mental health team members.
• Checks and monitors UHS email on a daily basis and responds in a professional and timely manner.
• Attends supervision on time.
• Facilitates treatment plan meetings every 180 days for each consumer on their case load.
• Completes treatment plans within 10 days after treatment plan meeting and prior to expiration date.
• Maintains appropriate communication with direct supervisor including returning phone calls/emails/texts within a timely manner and notifying supervisor of leave prior to the absence.
• Makes face to face contact with families at least one time per month.
• Displays knowledge of interventions and implements them in accordance with the treatment plan and with fidelity to the practice principles.
• Assists family to locate and appropriately use community resources, agencies, organizations and team members to coordinate services and monitor progress toward treatment goals.
• Completes and submits all documentation in accordance with specific deadlines, including but not limited to progress notes, assessments, discharges and service referrals.
• Completes and submits unusual incident reports and follow-ups as required by supervisor.
• Ensures optimal communication by relaying pertinent client care information to the supervisor.
• Ensure completion of required annual assessment which includes transportation as needed.


Master's Degree in Allied Health required with applicable active licensure in the District of Columbia (LGSW, LGPC, LICSW, LPC)

Two years working experience in a clinical field with at-risk youth preferred

Must possess a valid driving license and insurance and reliable transportation

Must possess knowledge of community resources and how to access such resources; family dynamics; crisis intervention/management; and strong organizational and communication skills.

Makes a positive first impression with clients, team members and outside agencies and presents a professional image – as reflected in the employees’ work attire and demeanor.
