Positions in this function include those responsible for providing process and operational support to the functions within Clinical Services. Positions in this function may also be responsible for audits, patient contact by face to face while member is confined in a hospital or skilled nursing facility and member outreach, discharge planning, making post - discharge follow - up appointments. Additional responsibilities include identification of members, verification of Health Plan eligibility and benefits, opening of clinical cases with preliminary information, attaching faxes to clinical records, entry of clinical service authorizations for medically necessary clinical services, tracking data by facility, and assisting with the inpatient appeal process.
***Onsite at assigned facility 3 - 5 days per week - NO TELECOMMUTE.

Primary Responsibilities:
  • Data entry related to admissions, discharges and referral requests into online system as well as processes existing requests within established timeframes
  • Verify benefits and eligibility
  • Maintains data entry of logs as required
  • Efficiently navigates and utilizes computer data entry programs not limited to: Practice Connect, Microsoft Office Outlook, Word, and Excel, Right Fax, Adobe Standard for fax attachment process
  • Onsite at assigned facility 3 - 5 days per week
  • Face to face meetings with members and family while confined in facility
  • Assist with discharge planning as needed
  • Schedules post - discharge follow - up appointments with members’ primary care physician as well as specialty physician as needed
  • Conducts Post Discharge Calls to members as assigned
  • Effective communication skills with internal and external providers and health plans
  • Actively participates in inservices, training, staff meetings, and other departmental meetings / committees consistent with Monarch HealthCare‘s core ideals and values
  • Processes appeals within required turnaround time as required
  • Maintain confidentiality of all member information and MHC business
  • Maintain professional demeanor at all times
  • Regular consistent attendance
