Primary Responsibilities:
  • Coordinate inbound / outbound calls - patient calls, appointments, DME, referral updates
  • Schedule, track and confirm various types of Nurse Practitioner (NP) home visits
  • Document outreach, NP visit activity and case information completely, accurately, and in a timely manner in EHR
  • Maintain and update team risk lists
  • Provide daily support  to NP’s in the field as needed (confirm addresses, phone numbers, reschedules, cancellations)
  • Identify and update demographics, member contact info and care levels in EHR
  • Fax NP encounters to PCPs
  • Fax patient orders to vendors (i.e. lab companies)
  • Notify pharmacies of new prescriptions and authorizations of refills as ordered and / or authorized by the Provider
  • Participate in quality monitoring process and closing gaps in care as directed
  • Demonstrate a high level of customer service behavior at all times
  • Expedite emergency calls as needed
  • Attend regular staff meetings as scheduled
  • Maintain confidential health information according to state and federal regulations including HIPAA
  • Assist with gathering and organizing of appropriate medical records (e.g. history and physical, lab work, test results)
  • Performs other administrative and care coordination duties as assigned by supervisor
  • Coordinates appropriate referrals to case management staff 
  • Demonstrates sensitivity to issues and show proactive behavior in addressing customer needs
  • Conducts urgency screens of new members and triages as needed
  • Monitors EMR for note completion and assigns patients to appropriate provider and program
  • Develops program reports for internal and external customer
  • Prepare materials for meetings
  • Coordinate supplies for providers in the field, ordering, distributing etc.
  • Gather and organize information for leadership to distribute to team
  • Perform data collection, data entry and quality monitoring activities, as applicable
Required Qualifications:
  • High School Diploma / GED (or higher)
  • 2+ years of telephonic customer service experience
  • Prior healthcare experience
  • Prior experience with medical terminology
  • Experience working with Microsoft Word and Microsoft Excel, with the ability to create / edit documents and spreadsheets
  • Previous experience utilizing a PC
Preferred Qualifications:
  • Associate’s Degree (or higher)
  • Experience working with chronically ill population
Soft Skills:
  • Strong Organizational skills
  • Strong Verbal and written Communication skills
