We're going beyond counseling services and verified referrals to behavioral health programs integrated across the entire continuum of care. In this position you will serve as a member of the care team, supporting case managers with care coordination and community resource identification in an effort to close gaps in care. You will interface with community-based organizations that assist with housing and food security, transportation, literacy support, utility support and others as identified within the care plan. Members who require assistance with identifying community resources will be identified from multiple sources and will be referred to you for assistance. You will utilize the telephone and email / text to connect with members and assist with their barriers to treatment. Other duties as assigned.


This position requires ACHA Level II Background checks (fingerprinting) by the State of Florida for all Clinicians that have face-to-face contact with members and will require renewal every five years.

Required Qualifications:
  • High School Diploma / GED (or higher)
  • 1+ years of experience working with individuals with a mental illness and / or substance use disorder
  • Experience with Microsoft Word (create correspondence and work within templates), Microsoft Excel (data entry, sort / filter, and work within tables), and Microsoft Outlook (email and calendar management)
  • Experience independently researching and connecting with community resources
Preferred Qualifications:
  • 6+ months of experience working independently with little supervision
