Primary Responsibilities

  • Engage members telephonically to coordinate services, community resources and treatment needs
  • Deliver a holistic approach to coordinated care based on the enrollee needs
  • Identify early risk factors and conduct ongoing assessments, and document in an electronic file
  • Function as an advocate for members and decrease unnecessary hospitalizations
  • Collaborate with the member's PCP to deliver and coordinate necessary services

Expect to spend about 8 hours a day on the telephone helping members from a variety of backgrounds. You’ll need to quickly coordinate the right services to meet a full range of behavioral needs, while working in a fast-paced environment.

Required Qualifications:

  • Bachelor’s Degree in Social Work
  • Professional License in good standing for respective discipline
  • 2+ years professional experience in a social work position with geriatric adults/Medicare population

Preferred Qualifications:

  • Bilingual Spanish
  • Knowledge of CMS regulations Medicare, Medicaid
  • Telephonic and/or call center experience
