Centennial Peaks is seeking a dynamic and talented Full Time Social Worker

Centennial Peaks Hospital is a 104 bed hospital located in beautiful Louisville, CO. We are dedicated to providing a full range of mental health and chemical dependency services to the state of Colorado. We strive to enhance the mental health, safety and well-being of our patients, their families, and the community at large by providing ethical, compassionate, medically and psychologically integrated inpatient and intensive outpatient treatment for psychiatric and addiction disorders. The ability to multi-task and thrive in a fast paced environment is critical. 

Our Social Worker provides clinical social work services on a designated unit in a manner consistent with the philosophy and objectives of Centennial Peaks Hospital. The Social Worker is responsible for psychosocial assessments, individual, group and family therapy, participation in treatment team planning, processing commitments, and discharge planning/case management service, crisis intervention and liaison services with referral sources and other departments within the hospital. Must have schedule flexibility, some weekdays and some weekends

Position Tasks: Performs Psychosocial Assessments Leads Group Therapy/Family Therapy as needed Develops and implements treatment plan Coordinates Transition and Discharge Planning Acts as liaison between patients, their families, and various organizations in mental health matters Clinical Responsibilities: Completes required social history within 48 hours of admission or makes appropriate provisions for completion. Provides regular family therapy in accordance with presenting needs and medical orders. Documents all family contacts and therapeutic activities in the record in a timely manner. Provides parent and family education as needed. Intervenes in crisis situations as needed. Counsels individual families and patients as needed. Actively participates in treatment team meetings and communicates all pertinent information to appropriate staff. Plans and documents all discharge activities in a timely manner. Intervenes in appropriate systems as a patient advocate. Leads groups as assigned. Involves patient and/or family in the treatment planning process and subsequent reviews.

Education:     Master’s degree in Social Work, Clinical/Counseling Psychology or Social Services related field required, licensure preferred.

Experience:   1 to 2 years mental health experience. Prior experience working with a psychiatric mental health inpatient setting preferred.

Licensure/Certification:         License eligible degree, LCSW, LMFT, LPC preferred;

Knowledge:      Prefer knowledge of case management, discharge planning, group leadership skills, age specific growth and development, crisis and behavior management, family therapy theory and practice.
