Position Summary:

First Home Care Inc. is one of the largest Mental Health providers in the Washington DC Metro area. We provide services that enable youth, families and adults to receive the care they need in their homes, communities and schools, reducing the need for hospitalization and out-of-community placement. Our programs offer free-standing, independent levels of care. As such, they provide ancillary treatment to support programs of Social Service agencies, special education programs and office based therapies. 


First Home Care has a long-standing management agreement with Foundations for Home and Community, a Maryland non-profit corporation. We are currently seeking Licensed Social Workers to join our Therapeutic Foster Care team in Lanham, Maryland.

There is a $1000.00 Sign-on bonus for this position!

Job Description:

  • Meets with the client and therapeutic foster care parents weekly in the home for the first 30 days of placement to ensure bonding and assesses any potential problems in the placement.
  • Provides crisis intervention evaluation and assessment services for clients and therapeutic foster care parents in the home environment once foster child is placed.
  • Provides information, education and guidance to therapeutic foster care parents, the biological families, or adoptive families of clients concerning issues of discipline, limit-setting, substance abuse, budget management, housekeeping, nutrition, independent living skills, communication skills and child development.
  • Advocates for, coordinates and links children, adolescents and their foster families with needed agencies in the community.
  • Ensures that the client, foster parents and biological families (when applicable) and others are involved in the clients’ treatment planning process. Ensures that copies of the treatment plans and quarterly updates are forwarded to all parties.
  • Assists the clients, therapeutic foster care parents and families in developing and appropriately using support networks. Provides on-call services to clients and foster parents as assigned.
  • Assists team in identifying goals and coordinates needed treatment services to meet these goals.
  • Keeps accurate and timely records and reports and completes intake assessment and social histories as scheduled in a timely fashion.
  • Provides emotional support and relationship building to therapeutic foster care parents.
  • Supports and enhances the clients’ relationship with biological family members when applicable. Arranges and encourages regular contact and visitation as specified in treatment plan. Keeps families and other team members updated on child/youth’s progress.
  • Assesses the clients’ progress, the family interactions, safety issues and stressors in the home environment.
  • Provides weekly contact by phone or in person with the therapeutic foster care parents.
  • Meets with client and foster care parents at least twice a month. Visits home at least once a month after the first 30 days of placement have passed until determined otherwise.
  • One to one with the client at least two times a month to assess progress, provide time for the client to express any concerns and to monitor for the presence of or potential for abuse.
  • Keeps the Supervisor informed of important information regarding the continued approval and management of foster homes for cases assigned to them.

 

 
Requirements

Requirements:

  • Master’s degree in Social Work and Maryland Social Work licenses required, (LGSW).
  • One or more year’s experience in case management with school systems, military agencies, social services, public and/or private community agencies, churches, civic organizations and/or the courts working with adolescents and/or children and families.
  • Must possess a valid drivers license, insurance and reliable transportation
  • Must possess knowledge of community resources and how to access such resources; principles of effective home, family and personal management; family dynamics; crisis intervention management; parenting and child care skills training; home management; resource/network building; and strong organizational and communication skills; assessment/treatment planning skills; understanding of family styles.
  • Must be knowledgeable of and abide by HIPAA regulations with regard to protected health information as defined by HIPAA. Uses and/or discloses on a daily basis verbal, written and/or electronic PHI, including use and/or disclosure in the performance of administrative duties related to the HIPAA privacy regulations as pertains to [CLIENTS’ FINANCIAL, DEMOGRAPHIC, AND/OR CLINICAL DATA] and/or [EMPLOYEES’ FINANCIAL, DEMOGRAPHIC, AND/OR CLINICAL DATA]
  • Required working hours per week are 40. Works in the home of therapeutic foster care parents and families of clients and the community requiring frequent local travel. Schedule will vary and include evenings, nights, and weekends.
