Position Summary:
Fremont Hospital is a 148 bed acute psychiatric hospital situated in the heart of the bay area. We are the largest freestanding psychiatric hospital in the East Bay filled with diversity and the feel of a close knit community. Fremont Hospital strives to be an integral part of the community, continually expanding our efforts to serve the psychiatric needs of adolescents and adults.

We currently seek an experienced Social Worker / Therapist (Clinical Case Manager) with MFT, MFTi, LSCW, ACSW, LPCC or MSW to fill our open part-time/on-call positions. The Social Worker will perform clinical services functions by providing ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of the individual patient needs. The overall goal of the position is to enhance the quality of management and satisfaction, to promote continuity of care and cost effectiveness through the integrating and functions of case management, utilization review, and management of discharge planning. Has accountability for the care, coordination and discharge planning of all patients on assigned unit(s).

The Social Worker will be responsible for completing the psycho-social assessment forms for each new patient and entering specific information on the treatment plans; interfacing with the physician led treatment team. Provides hospital case management/utilization review and discharge planning to assure that the patient progresses through the continuum of care and is discharge to the least restrictive environment.

In addition, the Social Worker will be responsible for coordinating the integration of the social services function into patient care, discharge, and home planning processes, interface with Fremont Hospital Clinical Staff, with outside referral agencies, with managed care organizations to ensure that the admission, continued stay, and discharge planning is in compliance with The Joint Commission (TJC), Center for Medicaid & Medicare Services (CMS), and established Discharge Planning Policies and Procedures. Provide oversight and coordination of legal paperwork and Lanterman Petris Short (LPS) regulations and conduct Local Coverage Determination (LCD) Audit as required.
 
Requirements

Social Worker I
Education:
A Bachelor’s degree for an accredited school of Social Services, Psychology, Nursing, Social Work, or Mental Health related field required. A Master’s Degree is preferred.

Experience:
Minimum 2 years direct clinical experience in a psychiatric or mental health setting and experience in case management/utilization review, discharge planning from hospital, patient assessment, treatment planning and communication with external review organizations or comparable entities is preferred.

Licensure & Certification:
Masters prepared and actively working on licensure, also registered for professional licensure by California Board of Behavioral Sciences preferred (MFTi or ACSW).

Additional Requirements:
CPR/AED Certification must be current and presented to Human Resources upon hire. You must successfully complete the Fremont Hospital’s non- violent Crisis Prevention Intervention (CPI) training course within 30 days of employment and prior to assisting with preventative intervention measures. Displays leadership, ownership and dedication to superior quality care.

Social Worker II
Education:
A Master’s degree from an accredited college or university in social work, mental health or a related field and active pursuit of licensure is required.

Experience:
Minimum 4 years direct clinical experience in a psychiatric or mental health setting and experience in experience case management/utilization review, discharge planning from hospital, patient assessment, treatment planning and communication with external review organizations or comparable entities is preferred. Must have two years of acute psychiatric in-patient therapeutic experience carrying an active case load is required, plus strong supervisory and leadership skills, and ability to resolve/manage conflict.

Licensure & Certification:
Masters Prepared with current and active license Issued by the California Board of Behavioral Sciences, or other applicable license: LCSW, MFT, or Licensed Psychologist is preferred

Additional Requirements:

Displays leadership, ownership and dedication to superior quality care. Will oversee and sign off on all clinical activities performed by Bachelor’s degree level and student interns.

Must be flexible to work various shifts during the week.
