As a Community Health Worker (CHW), you will act in a liaison role with AARP members to ensure appropriate care is accessed as well as to provide social assessments and member education.

Primary Responsibilities:

  • Create a positive experience and relationship with the member by engaging them telephonically
  • Proactively engage the member to manage their care and assist in decreasing barriers
  • Provide member education regarding community resources (financial, transportation, etc.)
  • Support transitions of care
  • Help to keep members compliant with their care plans
  • Partner with care team (community, providers, internal staff and caregivers)
  • Knowledge and continued learning of community cultures and values
  • Conduct member assessments if needed including functional and social needs
  • Assist with maintaining member engagement in conjunction with case manager
  • Document information (e.g., assessment information, member interactions, referrals, follow up plans) in relevant computer systems
  • Other duties as assigned
Position Description:
  • High School Diploma / GED (or higher)
  • 2+ years of experience working within the community health setting in a Healthcare role
  • 2+ years of experience working with community resources and support
  • Ability to create, edit, save, and send documents utilizing Microsoft Word and Microsoft Excel
  • Ability to navigate a PC to open applications, send emails and conduct data entry
  • Ability to obtain Community Health Worker (CHW) Certification within applicable states as required within one year of hire
  • Must reside within a commutable distance to the St Louis, MO office
Preferred Qualifications:
  • Bachelor's Degree (or higher) in Social Work
  • Knowledge of Medicare Regulations
  • Experience in Managed Care
  • Community Health Worker (CHW) Certification
