Primary Responsibilities:
  • Make outbound calls to assess members' current health status
  • Provide a complete continuum of quality care through close communication with members via telephonic interaction
  • Identify gaps or barriers in treatment plans
  • Partner with members and their providers to close gaps and prevent avoidable admissions / readmissions
  • Provide patient education to assist with self - management
  • Facilitate optimal health and well-being thru condition education, medication reconciliation, and connection to appropriate resources
This is high volume, customer service environment. You will need to be efficient, productive and thorough dealing with our members over the phone. Strong computer and software navigation skills are critical. You should also be strongly patient-focused and adaptable to changes.

This role is required to work in the Greensboro office on a daily basis for training (approximately 6 months).  Once the employee has been fully trained and is meeting goals/metrics, that employee is eligible to work from home.  If there are performance issues, the employee will be required to return the office. 

Required Qualifications:
  • Active, unrestricted RN license in the state of North Carolina
  • 3+ years of clinical experience as a Registered Nurse in any setting
  • Intermediate / proficient level of experience with MS Office applications
  • Passion and commitment to excellence
  • Positive attitude and solution-oriented
  • Ability to handle multiple tasks and function in a high volume environment
  • Ability to work well as part of an interdisciplinary team
  • Must be willing / able to obtain CCM within 2 years of employment
  • Must be able to commute to the Greensboro office on a daily basis

Preferred Qualifications:

  • BSN
  • Experience in managed care, case management and / or disease management 
  • Certified Case Manager (CCM)
  • Knowledge of disease processes for chronic illnesses in the senior population
  • Home Health experience
  • Knowledge of Medicare guidelines and regulations
  • Bilingual in English and Spanish
