Job Description

The Televideo Assessment Team Evaluator (“Assessor”) provides mental health assessments for contracted emergency rooms and/or hospitals, recommends the appropriate level of treatment based on the Assessor’s assessment of risk factors, and triages patients to appropriate services.

 

ACCOUNTABILITIES:

  1. Independently assesses mental health of patients in contracted emergency rooms and/or hospitals via televideo conference.

  2. Completes documentation related to the mental health assessment of patients.

  3. Determines potential of risk to patient and/or third parties based on individualized televideo assessment.

  4. Relies upon clinical training and acumen to determine the most appropriate course of treatment/level of care for patient.

  5. Provides treating physician and ER Supervisor with level of care recommendation.

  6. Discusses patient’s status, history, and level of care recommendation with family members, if available.

  7. Counsels patients regarding recommended level of care; and provides at least three (3) referral choices to patient when the recommendation includes a inpatient/outpatient psychiatric care

  8. Works with QR Intake and ER RN/Director to coordinate transfer to facility.

  9. Provides ER/hospital with copy of completed assessment form and recommendations.

  10. Tracks and submits list of patients seen, time of televideo assessments and disposition of each matter to supervisor.

  11. Adheres to all QRBH and hospital policies and procedures.

  12. Adheres to all hospital service excellence standards.

  13. Performs other duties, as assigned.

 

CUSTOMERS SERVED

Patients of facility, their family members, and all other hospital personnel. 

 

Job Requirements

To perform this job successfully, an individual must be able to satisfactorily perform each of the essential duties/primary responsibilities listed above and be comfortable utilizing video technology to confer with patients.  The responsibilities listed above are representative of the knowledge, skills and ability required.  This position requires the skill and ability to assess patients’ needs for therapeutic services, calmly handle high-stress situations, and make appropriate recommendations.  The position requires in-depth knowledge and specialized training in mental and behavioral health issues, and it requires meticulous recordkeeping and documentation.

 

EDUCATION

Master’s degree in Social Work, Psychology, Counseling, Behavioral Health or related field.

 

LICENSURE/CERTIFICATION

Current license, active and in good standing, in the State of Arizona as a Licensed Professional Counselor (“LPC”) Clinical or Social Worker (“LCSW”) preferred, or LAC or LMSW.

 

Experience

Minimum of 2 years’ experience in mental health

Additional Requirements: 

Successful completion of CPR Certification and successful completion of Handle with Care prior to assisting with a restraining procedure and within 30 days of employment.
