The Program Therapist provides Social Service activities and specific treatment goals to targeted patient population as delegated by the Social Services Supervisor and/or Clinical Services Supervisor. ESSENTIAL JOB DUTIES/RESPONSIBILITIES I. TREATMENT PLANNING On assigned therapy cases, is able to formulate clearly defined goals and objectives as evidenced by writing goals that reflect the patient’s presenting problems as seen in the assessments. Is able to accurately reflect progress of the patient specific to the treatment goals. Treatment plans, updates and final treatment plans are completed timely, per hospital guidelines. Conducts Patient Care Monitoring as seen by monitoring the requirements for each patient and initiating a PCM when needed. This document is thorough and specific and provides guidance for treatment. II. ASSESSMENT OF PATIENTS Ability to demonstrate knowledge of developmental tasks of children and adolescents. Ability to demonstrate knowledge of developmental tasks of adults. Demonstrates the ability to recognize signs and symptoms of anxiety and impending behavioral escalation Ability to recognize and evaluate behavior, changing levels of depression, tendencies toward self-aggressive behavior, cognitive functioning deficits, learning deficits, and patient’s capacity for self-care. The social history is completed within 48 hours of admission as evidenced by documentation in the medical record. III. TREATMENT TEAM Accurate information is communicated to all members of the team as observed by the supervisor. Works cooperatively and positively with all treatment team members as observed by the supervisor. Is flexible and creative with job requirements and expectation as observed by supervisor. IV. TREAMENT DELIVERY Individual therapy occurs at least twice per week and is documented in the patient’s record within 24 hours of each session as evidenced by documentation in the medical record. Multi-family and/or family therapy occurs as required by needs of the case and is documented in the patients medical record within 24 hours of service delivery as seen by a family therapy note in the patient’s record. Therapy notes reflect an awareness of the patient’s pathology and problems, notes are goal directed and follow the prescribed format (P.I.E.). This is evidenced by the notes in the medical record. V. DISCHARGE PLANNING Therapist begins discharge planning at admission as seen on the social history and treatment plan. Assists families in securing appropriate aftercare as evidenced by documentation Completes the aftercare form (Multi-Disciplinary Discharge Planning / Treatment Team Recommendation) timely and thoroughly and provides this information to the parent/guardian/patient prior to discharge.
