Wellness Coordinators provide telephonic care coordination and assistance with locating needed clinical and community resources to ensure successful outcomes and maintain a high level of customer service. They are responsible for entering relevant data and clinical information into multiple platforms.

Primary Responsibilities
  • Educate / address HEDIS / Out of compliance issues with Inpatient / Outpatient providers
  • Educate providers / UHCCP staff on the discharge summary requirements and discharge related processes
  • Assist providers / members with resource linkage when necessary to meet specific clinical needs, availability issues or geoaccess issues
  • Collaborate with all Interdisciplinary teams as needed to address discharge planning issues as well as other member’s needs
  • Assist members with rescheduling follow up appointments, troubleshooting discharge prescription issues and assisting with transportation and / or other resources that may prevent re hospitalization
  • Assist with discharge reports and complete assessments as needed
  • Process member discharge information into clinical system
  • Demonstrate initiative in achieving individual, team, and organizational goals and objectives
  • Other duties as assigned
